Zygomatic Implants for Severe Bone Loss: A Life-Changing Alternative to Dentures and Bone Grafts

Imagine being told that dental implants aren’t possible unless you go through multiple surgeries, bone grafts, or long recovery periods. It’s a situation many patients face — especially those who’ve worn dentures for years or have severe bone loss in the upper jaw.

That’s where zygomatic implants come in. They may not be as well-known as All-on-4®, but for patients with severe bone loss in the upper jaw, zygomatic implants can restore a confident, fixed smile — without the need for extensive bone grafting or settling for dentures.

How Zygomatic Implants Support Full Mouth Teeth Replacement

If you’re missing most or all of your teeth — or know you’re about to — you’ve probably started searching for alternatives to dentures. Traditionally, full mouth dental implants involve placing 6-10 implants per arch to support a complete set of replacement teeth.

A more advanced and streamlined version of this approach is the All-on-4® treatment concept, which uses just four strategically placed implants to support a fixed, natural-looking bridge. It’s ideal for many patients, even those with moderate to severe bone loss.

However, if the bone in the upper jaw has deteriorated significantly, even the angled implants used in All-on-4® may not provide enough stability. Around 10% of our patients fall into this category of extreme bone loss.

This is where zygomatic implants offer a breakthrough solution — anchoring into the cheekbone (zygoma) to support a full upper set of teeth without the need for bone grafting.

What Makes Zygomatic Implants Unique

Yes, zygomatic implants are longer and anchored into the cheekbone — but that’s just the start. What really sets them apart is their ability to restore smiles in patients who have been told they don’t have enough bone for traditional implants. When other solutions aren’t possible, zygomatic implants often become the game-changer — offering stability, strength, and hope where others fall short.

How Zygomatic Implants Work When Jawbone Is Not Enough

The zygoma (cheekbone) consists of dense hard bone, significantly stronger and more stable than the upper jawbone. Unlike the jaw, it’s largely unaffected by the bone loss that typically follows tooth extraction, making it an ideal support for zygomatic implants.

Zygomatic implants are designed to bypass the weak or missing upper jawbone entirely and engage this strong, reliable anchor point. That’s why they’re ideal for:

  • Patients who’ve worn dentures for 10+ years

  • People with failed bone grafts or failed implants

  • Those who’ve been told they’re “not suitable” for dental implants

A Surgical Approach That Requires Mastery — Not Just Experience

Placing zygomatic implants isn’t just about using longer implants. It’s a completely different surgical technique that requires:

  • Advanced training in facial anatomy

  • Precise understanding of the sinus cavity and zygomatic bone shape

  • Use of digital 3D planning software

In most cases, the implants are placed at a sharp angle, usually between 30 to 50 degrees, and come out further back in the mouth compared to traditional implants.

This angled approach helps avoid areas where bone is thin, but it also means the zygomatic implants pass close to important structure of the face, like the eye socket, infraorbital nerve and sinus cavity.

That’s why precision is so important — even a small mistake can lead to unwanted complications.

Types of Zygomatic Implant Configurations

Zygomatic implants aren’t always used on their own. In fact, they’re often combined with regular dental implants to give you four stable anchor points — just like in a typical All-on-4® procedure. They’re used as a substitute when there’s severe upper jawbone loss, where standard implants won’t hold.

Here are the most common configurations:

  • Quad Zygoma (Four Zygomatic Implants): Two zygomatic implants on each side of the upper jaw (four in total). This is used in cases of the most extreme bone loss, where there’s no bone left in either the front or back of the upper jaw.

  • Double Zygoma (Two Zygomatic Implants): One zygomatic implant on each side at the back of the jaw, combined with two regular implants in the front. Ideal for cases where there’s still some bone at the front but not enough at the back.

  • Single Zygoma (One Zygomatic Implant): Used when bone loss is only present on one side. One zygomatic implant is paired with three regular implants to complete the full set.

This flexibility allows experienced surgeons to design a solution that’s tailored to your unique anatomy — restoring strength, comfort, and function even in the most challenging cases.

Immediate Function — Made Possible Through Precision Planning

One of the biggest advantages of zygomatic implants is that they often allow for immediate loading — meaning you can receive a fixed set of teeth within 1–3 days after surgery.

But this isn’t automatic. Immediate function is only possible when:

  • All implants achieve excellent stability during surgery

  • The bite force is evenly distributed across multiple implants

  • Surgical placement is planned with the final prosthetic outcome in mind

At The Point Dental, we meticulously design every case digitally before surgery. This allows us to simulate bite forces, ensure a precise prosthetic fit, and even preview the aesthetics of your future smile — all before making the first incision. It’s how we deliver predictable outcomes, even in the most complex cases.

Patient Experience: What It Really Feels Like

Patient before and after

From a patient’s perspective, the idea of zygomatic implants can sound intimidating — especially when you hear words like “cheekbone” or “long, angled implants.”

But here’s what surprises most people:

  • The entire procedure is done under general anaesthesia, so you’re fully asleep and cared for by a full medical team.

  • Recovery is often no more difficult than with conventional dental implants.

  • Pain is typically mild and managed with pain medication.

  • You walk away with a fixed set of teeth within 1–3 days after surgery.

In fact, if we didn’t tell you whether you received standard or zygomatic implants, you wouldn’t be able to tell the difference.

For many patients, the most emotional moment is when they finally look in the mirror and see a full, confident smile — often after years of hiding behind dentures or struggling with failing teeth.

Understanding the Risks and Success Rates of Zygomatic Implants

Yes, zygomatic implants are more complex, and like all procedures, they carry risks.

These may include:

  • Sinus-related issues

  • Minor bleeding or swelling near the cheek area

  • Prosthetic complications if planning isn’t precise

That’s why this procedure should only be performed in settings with:

  • Highly qualified and experienced surgical teams

  • Access to on-site general anaesthesia

  • Full digital planning and 3D imaging

  • A clear post-op recovery and maintenance protocol

At The Point Dental, we’ve performed zygomatic implant procedures for close to 10 years with outstanding long-term results, with over 99% success rate — and many of our patients came to us after being told “you have no options.”

Are Zygomatic Implants Worth It?

If you’ve lived with the limitations of dentures, or the frustration of being told you’re not a candidate for dental implants, zygomatic treatment can be absolutely life-changing.

You’re not just getting implants — you’re getting:

  • A functional bite

  • A stable, fixed smile

  • Freedom from adhesives, removables, and embarrassment

  • And most importantly — hope

Dr Wilfred Koon’s Vision at The Point Dental: Smiles Without Limits

Dr Wilfred Koon, our principal surgeon, believes that beauty should never be a luxury — it should be accessible to everyone, regardless of how complex their dental journey may be.

After graduating from dental school, he pursued advanced training in implantology, driven by a passion to help patients who felt out of options. When he discovered the All-on-4® treatment concept, he saw not just a breakthrough technique, but a second chance for those who had lost hope.

That same vision inspired him to push the boundaries further by mastering zygomatic implants — an advanced, highly skilled procedure designed specifically for patients with severe bone loss who may have been told they are “not suitable” for implants.

Dr Koon is now proudly one of only a few surgeons in Australia qualified to perform this life-changing treatment.

At The Point Dental, this philosophy drives every case we take on:
“Everyone deserves to feel proud of their smile — no matter how complex the journey.”
